Just back from night duty! Yes, I left at about 8.40pm. It's not really night duty, since we followed the regs and cons for the exit round. It lasted till 8!! OMG ... tonnes of new admissions tonight, more than 15 I think. We travelled a few rounds to the same wards - first we did 36, then went up to 38. Just as we were finishing 38 there'll be some new admission at 36, and back we'd go again! Saw some very interesting lacerations - chunk of finger cut off with blood still dripping - 2 of these, actually, calcaneal #, 2 # neck of femur - a Garden's grade 4 and a grade 3. The grade 3 was very tricky. It looked like a grade 4 until you realise that the head of the femur isn't snugly fit into the acetabulum, which means some of the ligaments are still attached from the head to the neck. Hmm .. and some interesting stuff like pigeon toes, hip #, and also fat necrosis. The woman's a bit cuckoo ... schizo, and had past Hx of attempted suicide. She kept insisting she had pus in her knee, but it's actually just fat. Very very young woman too. Makes you wonder what kind of traumatic life experience they've had to actually turn out like this. Ooh, and also visited the prison ward! Nice opportunity, since the doc was like the patron saint of that ward and the wardens let all 6 of us in (3 year 5s). The poor man had a huge piece of glass stuck to the anterior aspect of his joint, and it looked really bad. Also a man who somehow tripped over something and tore the entire big toenail off the nailbed.
N and PBP stayed on to see Dr. R do a debridement of the man with the torn nail and an attempt at fixing the nail back. Decided to leave early cos it wasn't really relevant. Dr. R was also very tired and he told the M5s he can't give them a tut cos he's too tired, so I suppose the chances of actually learning anything is quite slim.
Then dad called just as I was leaving. 49 year old Chinese gentleman presents with 10 mins of chest discomfort, so he decided to go to see our GP instead of the A&E. Took an ECG and just called me, doesn't seem to be an AMI. Think he mentioned that the pain radiated to the back .. pancreatitis? Hope not .. it's a nasty thing to get.
Oh, I must gossip about one of the M5s. He's pretty disgusting actually. His whiskers are AT LEAST 3cm long ... has he not shaved for years?!? Yucks.
And 3 people from *cough* the other group *cough* had night duty too, clashed with ours!! Then they came up with some i dunno what explanation ... oh well. At least they cleared off somewhat, I actually felt a little sorry for them but seriously. They could have just gone home instead of hanging around, couldn't they? Or maybe I'm still a little sore about what happened to my other groupmates.
Feeling bad about leaving N and PBP alone .. I've been pretty much alone today, and left them to clerk as well. But the thing is, as Mr. Ye pointed out when I confided in him what was disturbing me, it's probably because I need more personal space. More "me" time, to finish up things I think need to be done first. Really hope they didn't think otherwise, maybe like I'm AS or something.
